Other duties include working closely with SNAP Employment and Training vendors to move clients to self-sufficiency. The incumbent will counsel, case manage and make referrals regarding client assistance with benefits DSS does not provide such as Child Support, foreclosure assistance, or food closets. Case Managers will be responsive to the diverse needs of vulnerable and low-income residents by sharing information in addition to providing timely and accurate benefits that improve nutrition and health.\r\nEssential Functions\r\n\r\nEssential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code, Title 24.�\r\n\r\n\r\n\r\n\r\n\r\n Interviews clients, family members, and others to determine eligibility and connect clients with health and human services.\r\n \r\n Develops a service plan to aid and assist a client in receiving care and assistance needed in collaboration with hospitals, clinics, agencies, and other appropriate resources (Service plans may include job training and employability, housing, general public assistance, in-home services and supports or home-delivered meals, de-escalation techniques, peer services, or skill development).�\r\n \r\n Evaluates information derived from client interviews, case history, and other pertinent sources and refers to proper channels for review.�\r\n \r\n Provides guidance and relevant information to clients and families prior to receiving requested service and assistance.\r\n \r\n Coordinates with other agencies, hospitals, clinics, and resources to expedite the process on behalf of the client.�\r\n \r\n Prepares case histories and reports and maintains records and chronological logs of case activities.�\r\n \r\n Uses automated information systems and computer software to research, review, evaluate, and report on health/human service data.�\r\n \r\n Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.�\r\n \r\n Performs other related duties as required.\r\n\r\n\r\n\r\n\r\n\r\n\r\n\r\n \r\n \r\n\r\n\r\n\r\nJob RequirementsJOB REQUIREMENTS for Health/Human Service Case Manager I\r\nApplicants must have education, training and/or experience demonstrating competence in each of the following areas:\r\n\r\n Possession of an Associate’s degree or higher in Behavioral or Social Science or related field.\r\n\r\n OR\r\n\r\n Three years’ experience in health or human services support�such as�interviewing clients and assessing personal, health, social or financial needs in accordance with program requirements�or coordinating with community resources to obtain client services.\r\n Six months’ experience in making recommendations as part of a client’s service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.\r\n Six months’ experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.\r\n Six months’ experience in narrative report writing.\r\n\r\nConditions of HireThis position is a classification organized under an\r\nexclusive bargaining representative (labor organization) that has been elected\r\nby employees as their representative for collective bargaining and other applicable\r\nterms and conditions of employment, in accordance with Title 29, Chapter 59 and\r\nTitle 19, Chapter 13 and 16.� This\r\nposition is covered by a collective bargaining agreement based on its\r\nindividual terms.\r\n�\r\nApplicants must be legally authorized to work in the United\r\nStates.
