  • Maintain an adequate inventory of prepared charts ready for admissions
  • Assign medical record numbers for all new admissions and maintain medical record number log
  • Prepares and maintains the diagnosis list and proper diagnosis codes
  • Prompt completion of the medical record face sheet following admission. Update responsible party as needed
  • Audit the charts of new admissions within 24 and 48 hours to ensure compliance of all admission aspects
  • Review the charts at the end of the month to ensure all orders have been transcribed to the following month
  • Provide medical record information and/or copies to insurance companies, and other entities in compliance with HIPAA regulations
  • Ensure Medicare A resident charts certifications are kept current
  • Track MD visits to ensure visit schedule is maintained in compliance with regulations
  • Completes discharge record in a timely manner following discharge
  • Maintain charts in proper chart order
  • Thin charts as needed and under the direction of the Director of Nursing
  • Maintain proper handling and storage of charts following resident discharge
  • Ensure all medical records are maintained in a confidential and secure manner at all times
  • Prepares all medical records for closure, physician signature and completion of discharge summary.
  • Maintains confidentiality of all medical record information
  • Assist Health Services or Nursing Service Directors with chart auditing as requested
  • Maintain a work station that is orderly and records can be readily accessed

  • High School diploma or equivalent
  • Excellent interpersonal and communication skills and the ability to work with staff and external constituents.
  • Prefer medical records experience in a long term care setting.
  • Moderate computer experience required.
